Still a lot of discussion going on about the reasons for a "Blowout" to occur after inquires into the events of:
- Montara Spill in Timor Sea, Australia,
- the Macondo Disaster in Gulf of Mexico in the USA and
- the Lisu Mud Volcano in Surabaya, Indonesia.
The President of the USA established a National Commission to determine root causes and the Commission stated "From the outset, the Commissioners have been determined to learn the essential lessons so expensively revealed in the tragic loss of life at the Deepwater Horizon and the severe damages that ensued. The Commission’s aim has been to provide the President, policymakers, industry, and the American people a clear, accessible, accurate, and fair account of the largest oil spill in U.S history: the context for the well itself, how the explosion and spill happened, and how industry and government scrambled to respond to an unprecedented emergency."
Indeed on Montara and Macondo, the relief well and mitigation of surface oil reaching the shores went without loss control type miss hap.
Why did the emergency response operations go without a loss of life or loss control of the relief wells offsetting the wells blowing out?

Could an evaluation of the project coordination on the relief wells reveal the reason for the loss control on the original wells?
There is a common denominator or a common root cause in the individual loss control events listed above. How will the root cause be revealed?
