Non-technical skills in well operations

How much is currently known about how non-technical skills are used (or abused) during well operations? The IOGP are conducting a web-based survey to examine the extent and use of non-technical skills by safety critical roles of Drillers, Toolpushers, and Drilling Supervisors. The survey, which is completely anonymous, is available at: https://www.surveymonkey.com/s/iogp_baselinesurvey. We would ask all Drillers, Toolpushers and Drilling Supervisors to complete the survey before 1 May 2015 to voice your opinions.

I came across some comments around a blowout and surface casing breach in Bass Strait Victoria in Australia 1968. Platform to be decommissioned soon.  I read an ancient non technical comment on the reasons for the blowout.  Can we say what would be said by an experienced drilling engineer?
