Abstract

This meeting is intended to establish expectations about the unique, but cooperative, roles of engineers, safety professionals, and operations personnel in conducting unfamiliar, hazardous, and/or technically challenging operations safely.  It will begin with a recap of a SPE Distinguished Lecture that highlighted the deadly hazard inherent in conducting an air foam well cleanout that was only understood by those involved after a careful technical investigation of the incident.  Interactive discussions of field operations that are examples of both success and failure will be used to engage audience members in considering the roles that they could have played in each situation.  Hopefully, we will reach conclusions about our roles and our responsibilities to each other in conducting challenging operations successfully and safely.
